Mike Foley, owner of Emerginnova, joins me today to talk about ceramic zirconia implants. If you’re not familiar with it, Emerginnova is the US distribution company for the ZERAMEX Ceramic Dental Implants. As a listener to the show, you’re probably very familiar with titanium implants, but I think this conversation will be an eye-opening look into the world of ceramic zirconia implants.

Mike has been in the dental implant business for 15 years, and jokes that once you get into it, you can’t get out. Let’s analyze the word general.  It’s the opposite of specific.  It means average, non-specific.  Now let’s look at general dentistry.  It’s easy for the public to think all general dentists are the same and it’s therefore easy for the market place to commoditize this sector of healthcare service.   

You can see now how many dental offices are much like McDonald’s franchises or Starbucks. If they all appear the same to the general public and if a dentist, is a dentist, is a dentist, then what other choice do people make their decision from other than local convenience of geographic location, availability of hours, and price.  

The other option is to become specific, even super specific.  I have noticed that the riches are found in the niches. The more specific you can be to serve a certain client who is willing to pay you a Quality price for a quality service, then this is the way to be able to charge a fair price for your services and for providing more fulfilling dentistry. 

The specific procedures that you want to be known for should be based upon not only your talent but also your passions. It’s when you combine your passions with your skills when you’re able to do something special. People want to be treated special and are willing to pay for that experience. Not being everything to everybody is ok and actually better.   Instead let’s try to become something very valuable to someone who wants your services and is going to pay premium for them.  That’s where we can make our biggest impact.  If your passion and skills are implant dentistry then you are in the right place.   

How do you become specific is the next question?  We have to learn to make better things for specific people. Putting yourself in this category of one will enable you to understand and solve the problems they face and provide solutions and services they want.  Let’s do something remarkable together. I hear the question asked all the time, “What is the best dental implant to place for a general dentist”.  I always see the question answered form a surgically driven response.  Today I would like to take a wider look at the question and an even wider look at the answer.

A lot of what implant companies tell the clinicians and their marketing message is that their implants are superior and that once a clinician starts using their implants that to switch to a different Implant would be too difficult.

I would like to make the assumption that all implants made at a respectable manufacturing facility with a respectable process can and do work. 

So why do Dentists choose a dental implant system to start with and satay with that company line.  Most dentists choose implant systems based on where they received their training. 

If the industry of labs, dentists, Educators, and manufactures are all using a couple of systems that have open connections and parts then this will create a network effect.  It becomes easier and cheaper, and more useful to all parties to have industry standards so that processes and systems can become streamlined and universal.

I would also encourage dentists to look at who developed the system. Not all companies were built on the idea of general dentists training and using their products and continue to be dedicated to that idea and concept moving forward.

The final thing I would encourage dentist to look at while evaluating implant companies is: Are the implant companies that sell the systems private entities or publicly traded?

Today’s guest is Charles Schlesinger, DDS, FICOI. Charles is a dental implant practitioner and educator who has lectured domestically and internationally, and has been awarded as one of Dentistry Today’s Top Educators for several years running. He’s an expert in such topics as advanced restorative, implantology, and cadaver-based implant and grafting. When he’s not lecturing, he maintains a private practice in Albuquerque, New Mexico.

Charles’ name has been showing up everywhere for me lately, and I’m delighted to have him on the show. He’ll share his story and talk about where he did his studies, as well as how he got to where he is today. Given his expertise in dentistry, you would think this was his lifelong passion, but in fact he took a winding route to get here. He worked in immunology research for a couple years, then transitioned into teaching high school biology, before finally moving into dentistry.

As you’ll hear today, Charles believes that every dentist is capable of placing implants if they really want to. In addition to his periodontics training at the University of Texas Health Sciences Center at San Antonio, he completed an internship in oral and maxillofacial surgery and practiced for several years as a general dentist. He is a Clinical Assistant Professor in the Division of Periodontics at the University of Colorado School of Dental Medicine where he has served as a Course Director in both clinical and didactic courses. I get about 5-10 messages a day from people asking me which implants to buy, which CBCT to use, which surgical guides to use and other questions.

I thought possibly by setting up a group of general dentist friendly partnerships I could help introduce good products and organizations with people who would be a good fit. The partners that I would be introducing into this GPO, group purchasing organization, that we are starting would be organizations, labs, implant companies, and manufacturers that are all relevant to implant dentistry, and are friendly to use for the general dentist,. I was tired of seeing DSO‘s getting discounts on products and the general dentist still struggling to make overhead and struggle to find quality systems at a good price that were willing to teach and train general dentists and not just specialists.

Join the Group Purchasing Organization

The general dentist has to worry about the specialist groups and the DSO groups and I thought why not a group for the general dentists. I also want it to be objective and have it be non-specific to just one implant company or just one process so there could be multiple options available. I want to help link and partner with good organizations, implant manufacturers, labs, and other ancillary products that will help bring good value and create long-lasting connections for general dentist learning to place dental implants.

Implantology can be an expensive field, whether with time away from your office taking CE, whether it’s the expense of the instrumentation and kits, whether it’s the expense of the increase overhead of products, or increased technological equipment like CTCT. I wanted to make sure the dentists that were serious about implants could put their time and money wisely into products and services that I could guarantee would work for them and then I could also show them how to integrate them into their practice successfully. I feel if I can save someone time and money and help them make introductions to companies while still providing a non-biased opinion that I could do some service for the implant community and so the group purchasing organization partner program has come from that idea.

The program is going to be a yearly subscription of $100 per month or $1000 per year upfront which will then provide discounts on implant products, CE Courses, and other services that a dentist would need in order to run a successful implant business. In addition to that there will be upcoming webinars and CE opportunities outside of the initial concept. This will also allow me the ability to continue to refine and create more content and relevant aspects for learning and sharing. I hope to continue to further push the limits of implant dentistry for the general dentist while giving back to people looking to get into implant dentistry. Notes on preparring for and capturing the CT Scan

If your patient is dentate, follow the above steps, and capture a CT scan with the patient opened biting on cotton rolls.

If patient is edentulous, and the existing denture fits well and the tooth arrangement is accepted, convert into a scan appliance (instructions page 6). Perform a dual scan using the protocol in our manual. If edentulous patients needs to establish tooth position and vertical, begin conventional denture steps to create a scan appliance.

Dr. Kenneth Judy is a 1964 graduate of Georgetown University and a 1968 graduate of New York University College of Dentistry. During and after his pre-doctoral studies, Dr. Judy was the recipient of several research fellowships. He then trained in surgical prosthetics with Dr. Isiah Lew at the New French and Polyclinic Medical School and Health Center in New York. Dr. Judy has authored over one hundred research and clinical articles, editorials and textbook chapters. He has lectured extensively nationally and internationally. 

Dr. Judy is a Clinical Professor of Implant Dentistry at New York University as well as a Clinical Professor in the Periodontal department at Temple University. He is a staff member of the Dental Implant Center at Montefiore Medical Center/Albert Einstein College of Medicine in New York and maintains a private practice in New York. 

Dr. Judy serves on the Deans Advisory council of Columbia University School of Dentistry and on NYU’s School of Nursing.  Dr. Judy is a member of over sixty organized dentistry, editorial, research, prosthodontic, and dental implant and organizations. Since 1979, Dr. Judy has served as chairman and then co-chairman of the International Congress of Oral Implantologists (ICOI), the world’s largest dental implant society.

Dr. Resnik is a one of the original faculty and founding members of the Misch Institute since 1988 and worked alongside Dr. Carl Misch for over 30 years. He is internationally known as a leading educator, clinician, author and researcher in the field of Oral Implantology and Prosthodontics. He received his dental education from the University of Pittsburgh along with his specialty degree in Prosthodontics, Surgical Fellowship in Oral Implantology, and a Master’s degree in Oral Implantology/Radiology. 

Currently, Dr. Resnik is a Clinical Professor of Oral Implantology and Periodontics at Temple University in Philadelphia, Adjunct Professor at the University of Pittsburgh School of Dental Medicine Department of Graduate Prosthodontics, faculty at Allegheny General Hospital in Pittsburgh and the Surgical Director and Chief of Staff of the Misch International Implant Institute. He is a consultant to numerous implant companies and laboratories. Dr. Resnik is a diplomate of the American Board of Oral Implantology and the International Congress of Oral Implantologists. He is a frequent lecturer, both domestically and internationally, along with being a leading researcher and author in the field of Oral Implantology. Dr. Resnik is the author of the recently released leading textbook in Implant Dentistry “Avoiding Complications in Oral Implantology” by Elsevier Medical Publishing. He maintains a private practice in Pittsburgh, PA limited to Oral Implantology.

Dr. Mark Bishara graduated from the UWO Medical Biophysics program and was selected as a gold medal recipient for being the top graduate of his academic program. Following this, he continued his studies at the University of Western Ontario dental school. Although he enjoys all aspects of dentistry, he has a very special interest in all surgical procedures including implant placement and full mouth rehabilitation. He also excels in working with children and looks forward to seeing families grow with him in our Bowmanville Dental office. With appearances on rogers TV and as proud member of the Bowmanville rotary club, Dr Mark looks forward to helping others in the community for years to come with our free dental day initiative hosted twice a year. In his spare time he enjoys playing basketball, painting on canvas and spending time with his wife and daughter Emily and son Jason. Some of his art work is featured in the office. 

Dr. Mark Bishara is also active as a lecturer with the Toronto Implant Academy on dental implants and is a  published author on topics such as PRF treatment and dental implants, as well as his contributions with the Canadian Implant Dentistry Network on FB and Homepage.

The Neodent system is the market leader in Latin America and holds more than 56% of the Brazilian market share. 

The system is known for its innovations, quality and technology. In Brazil, Neodent is a premium system, where we pay a premium price for it.  Here in US, due to the exchange rates, the system is offered in its premium quality but for a value price.  

Technically, Neodent evolved much through the years. Like most systems, we started with the external hex connection and moved up to the internal hex one. But what makes us stand out the most is definitely our Morse taper connection. It offers a lot of clinical advantages, especially when placed as recommended, such as bone and bone crest maintenance (with maintenance of the gingival papillae), higher mechanical stability (less risks of prosthetic screws getting loosen), a virtually "bacteria free" interface and preservation of the biological width, with a proper biologic seal with the dental-gingival fibers attaching to our abutments. That means protection for your bone-implant interface, with a well-defined probing depth and therefore a natural barrier against the excess cement flowing into the sulcus, on your cement-retain restorations. That also reduces the risk of peri-implantitis, since our implant surfaces are never exposed and instead, are covered by bone, sealed by dental gingival fibers and with no bacterial flow through the connection gap. 

Neodent also provides a hydrophilic surface called Acqua, which promotes more BIC in less time, improving the overall early osseointegration. 

The exterior designs of our implants are meant for gripping and giving the opportunity of immediate load in different bone densities. Also, regular, short, narrow, cylindrical, tapered, hybrids, zygomatics... They are all offered through the system so that we can always solve our diverse range of clinical situations.

Grant Bullis, director of implant R&D and digital manufacturing at Glidewell Laboratories, began his dental industry career at Steri-Oss (now Nobel Biocare) in 1997. Since joining the lab in 2007, Grant has been integral in obtaining FDA 510(k) clearances for the company’s Inclusive® Mini Implants, Tapered Implant System and Custom Implant Abutments. In 2010, he was promoted to director and now oversees all aspects of CAD/CAM, implant product development and manufacturing for more than 1,000 implant and prosthetic components at the lab. He has an MBA from the Keller Graduate School of Management.
